What to do in the event of the onset of an insurance case?

 

If an insurance case has arisen, you should contact:

and must complete a Claim application.


For your convenience, in this section you can discover information about the persons that are entitled to receive claim and about the additional documents which must be submitted together with the claim application in your specific insurance case:

  • At the end of the term of the agreement

    The following personal documents must be submitted:

    • a claim application;
    • an original copy of the policy;
    • a passport copy.

    If the documents are submitted by an authorised representative of the insured person, then those documents that certify the rights of this person to represent the recipient of claim must be submitted additionally.

     

    If the recipient of claim is a minor, then the claim shall be transferred into the child's bank account or paid out to the child's parents or the child's legal guardian, submitting the relevant decision from a family court that it permits the insurance claim to be received by another person who has reached the age of majority.

  • When terminating the agreement prior to its scheduled expiry date

    The following personal documents must be submitted:

    • a completed claim application;
    • an original copy of the policy;
    • a passport copy.

    If the documents are submitted by an authorised representative of the insured person, then those documents that certify the rights of this person to represent the recipient of claim must be submitted additionally.

    If the recipient of claim is a minor, then the claim shall be transferred into the child's bank account or paid out to the child's parents or the child's legal guardian, submitting the relevant decision from a family court that it permits the insurance claim to be received by another person who has reached the age of majority.

  • In the event of an accident

    The following personal documents must be submitted:

    • a completed claim application;
    • a copy of the policy, presenting the original copy;
    • a passport copy;
    • an extract from an outpatient's and/or inpatient's medical card that confirms the accident and which indicates the full diagnosis, examinations, treatment applied, course of treatment; examination results that confirm the diagnosis including X-rays;
    • if hospital treatment has been required – a extract from the inpatient's medical card;
    • copies of incapacity sheets for workers;
    • the insurer may request additional information from a medical institution or a copy of the outpatient's card.


    If the recipient of claim is a minor, claim should be requested and received by one of the child's parents by presenting the child's birth certificate.

  • In the event of the death of the insured person

    The following personal documents must be submitted:

    • a completed claim application form;
    • the original copy of the policy;
    • a passport copy;
    • a copy of his or her death certificate, presenting the original copy;
    • a document verifying the cause of death – a document issued by a medical institution in which the cause of death is indicated or a notice from a Registry Office regarding the cause of death;
    • if claim is due to be received by his or her heirs – a document verifying the inheritance rights of the individuals concerned.


    If the documents are submitted by an authorised representative of the deceased person, then those documents that certify the rights of this person to represent the recipient of claim must be submitted additionally.

     

    If the recipient of claim is a minor, then the claim shall be transferred into the child's bank account or paid out to the child's parents or the child's legal guardian, submitting the relevant decision from a family court that it permits the insurance claim to be received by another person who has reached the age of majority.
